At Aken Home Care LLC, our Wellness Coordination services help individuals with intellectual and developmental disabilities manage their health needs while living at home and participating in their communities.

Our licensed nurses work closely with individuals, families, caregivers, case managers, and healthcare providers to support coordinated, person-centered care.
